Actually, I'm studying medicine now. I can answer that. It's not so much the smoke is affecting the heart directly, but all the crap that can be in it will come into the bloodstream. Now, when this crap comes to the coronairy vains on the heart, they will be damaged. Then all sorts of crap will get stuck on the damaged part, closing off the vain. Then, after a while, the part of the heart where the vains would transport the blood to, will start to die off.
Usually, you get a cardiac arrest from that. But another thing that could happen, is something called Atrio-fibrillation, or ventricular-fibrillation. These are conditions that can be seen as a failing heart, or a weak heart.
