Perfect quarma / playing through again

I have a question about playing the game through more than once.

I just beat it tonight and chose the option to go through again and try to get perfect quarma. Will I have to play every single level again or just the ones that I didn't rescue everyone from?

All i know is that if you start the game again that you will have to save all the Mudokons again and go through all the regions again
