I am terrified of lifts.
I am also terrified of flying.
This friday i go to New York, where we are staying in a 50 floor hotel and of course we are on the top level. We also intend on going up the Rockafella center. Another 70 floors.
The minds of wisdom here should surely be able to come up with a suggestion...

Okay in all seriousness, there is no real way of getting over fears like this other then to just face them.
Also, since you're also afraid of flying, you might want to ask yourself if you don't have a case of claustrophobia. Fear of flying is often not caused by the flying itself but by the thought of being in a sealed off space with no direct way out. An elevator is the same principle, maybe even worse since it's even smaller and doesn't even have windows.
However, as a patch to your problem in the hotel there are two choices, one more healthy then the other.
1. Take the stairs. By the end of your stay you'll have the legs of a marathon runner.
2. To lighten your fear you could look at the ceiling and see where the escape hatch is. If you know there is a way out you might not be so stressed when you step in.
