I remember getting out of bounds in the area where you fight Sekto once. You know how you drop down from that corridor into the final room? If you get the jump right, you can land on the wall around the edge of that room, and then jump outside it.
Interestingly, while falling you get the whole “guitar-strum-and-fade-to-black” as when you die, but then you keep falling and eventually land on some pipes and stuff below. You’re forever stuck there.
