Actually that could work in the inverse sense.
A physical woman who looks and acts male could conceal contraband in the front of their trousers and merely look well endowed... Regular searches won't approach that area so if they act inconspicuous enough they could get away with it. And because their passport has no gender listed they would be able to pass as a slightly effeminite man.

They'd have to do a bloody good job of storing the contraband to make it feel like a real cock 'n' balls. Besides, you could do that with just about any not-massively-endowed guy.
