I think the problem in MO is that they tried too much to keep ideas and concepts from their 2D games and tried to make it work in 3D. And one hit death would have been impossible in MO the way it is now.

I don't really see how you came to that conclusion - MO is a
lot more action-oriented than AO/AE, and there's a lot less stealth - sneaking is essentially useless and there's no shadows.
The puzzles in MO were a lot more simplistic as well, mainly due to the above points.
The main thing that bugs me though is the change in tone and art direction.
