Just whipped the attached icon up for you using a quick
online tutorial for making icons. Consider it a prototype if it's not entirely what you're looking for.
It works on my machine but since Vista used a "zoom" function to generate the different resolutions for icons I'm guessing Windows 7 does the same... Either way I've done what I can to assure that all the resolutions you requested are facilitated.
