It looks to me like it's been photoshopped in. Which begs the question of what was originally in that photo that has been covered by the toothbrush...

FINALLY. Actually there was nothing. I was just doing ramdumb photoshop forgeries one night when she was over and put that toothbrush in front of her face since she was standing in front of the bathroom mirror
