In the Save One Bullet achievement, do shotgun shells count as bullets?

Yes. Secondary fire on the shotgun counts as two bullets. But it doesn't matter; the one bullet you fire is the only one in the game that you MUST fire (to open a door), so you can't shoot any more after that.
