Perhaps your system is dusty on the inside or something to that effect? Try using some Dust Off on the disc tray, and slightly on the inside of the system. Sometimes, a dirty or dusty system can interfere with gameplay.

I think it might be because of the system. And to answer Max's question, it did this to me twice.
