I think part of it might be pshycological...and no matter how many times you go under the knife, that won't go away.

I certainly agree with that for some cases. That's how many people get addicted to the surgery. They fail to realize the problem lies within them rather than on the surface.
Hmm, even for when surgery is chosen simply to change a feature that isn't necessarily causing emotional pain, I'd bet becoming addicted is equally easy. For example, if someone got a nose job that came out wonderfully, then he or she may start thinking, "Well, they did such a nice job on my nose, maybe they could make this look better... and then that! And then that!"
