If he can't get the operation, then what happens? I thought it was a life/death thing?

When Stranger said that he needed it to survive, he meant it as a metaphor. Steefs usually get shot down on sight, and Stranger gets shot at enough in his line of work...he doesn't need any more problems. So he asks the good doctor to graft the back of his hind legs to the front of his forelimbs *which, in reality, wouldn't work anyway, but neither would a high-pressured bounty cannon...* so he would have two legs and look fairly normal.
