I also remember one in Ratchet and Clank. It was at Blackwater City where you have to swim through the tunnels as the water is rising and somehow I ended up going through the roof of the tunnel and into the sky!! I was actually swimming in the sky!!! It was so weird!! Eventually I ended up dying.
