I didn't do any of that stuff to get the extra multiplayer map. I just beat the game. My Halo 2 is Limited Edition though, if that makes any difference.

No, it doesn't change a thing. My friends all have the LE, and the same thing happened to them where they didn't get it. And, it's a glitch, so that's probably why it randomly affects some, and not others.
