Hobbit:Podo or Samwise Bumbleroot of Fair Downs, again depending on whether it's the full or abbreviated version, only this time the full version gives the shorter name.

Elven:Angrod Táralóm
Hobbit:Gorbulas Gamwich of the Bree Gamwiches

It seems to go on the first letter of your first name and the first and last letters of your second name.
