Aliens vs Predator - the first level with the Marine when you first encounter an alien. It's just after a pitch black room with a lift, and you look down a smoky dark corridor and can hear this screech and see this bloody thing running toward you only to pounce unnaturally far away...cor.

Saying that HL2 when you go to that village with the priest bloke (cant remember his name) and those screaming things run at you and pounce...was pretty intense too.

Oh while I'm here - Fatal Frame 2 scared the crap out of me and my friend as well...that camera obscura flashing blue for a second then going black before a load of ghosts appear and come at you. Ouchy...
