Stupid people do stupid irrational things. Don't worry about them too much.

Guess you're right with that. People are weird and get pissed over the tiniest of things.

Short of asking them about everything I do, I wish there was a way of knowing what actions would offend who, and why.

Um... well.. You should have asked for permission. If you didnt have permission to use her voice, then I understand fully that she got offended.

Now if she did sayit was ok, and then changed her mind, then it was pretty shitty of her.

But all in all... I dont think its its illegal to put someones voice out there, since you cant really identify a persona that well.
Maybe you could edit out her voice or something.
If shes someone you dont care about you could put the vid out there anyway

Or you could make a new video.

Record yourself saying, in a slightly high pitched voice, 'I'mma cunt, I'mma cunt.' then layer this new sample over her voice. Then she can't complain about her voice being there but knows exactly what she is.

Seriously, I wouldn't think too much of it. I would take down the video and cut the toerag from my life. If she wasn't able to appreciate the work and time you put into that video all because she hates her voice then she either needs to stop talking or a kick in the arse.

I thought this was going to be a blog telling me to piss off...phew.

Don't worry about this, at the end of a day, you don't know her, and it's a youtube video (or something) just move on and don't worry so much about offending people.
